The Learning Better Together Coach supports process improvement initiatives under the Learning Better Together (LBT) framework to enhance patient centered care and patient-centered care and operational excellence across Open Door Community Health Centers (ODCHC). This role works closely with teams to streamline workflows, improve quality, and sustain ODCHC's Model of Care (i.e., puzzle pieces) by monitoring, developing, implementing, and refining standard processes. The coach provides guidance, training, and hands-on support to staff, helping them develop problem-solving skills and adopt best practices for continuous improvement. This position also tracks key performance indicators (KPIs), analyzes trends, and helps teams remove roadblocks that may slow down progress.
Compensation Range:
$30.00-$34.84
All new hires will begin at the base wage of this position.
Hmong and Spanish speakers who successfully pass a language exam will receive an additional .75 hourly differential to their wage.
