Responsibilities / Required Skills

• One of the following CA Licenses: Nurse practitioner (NP), Physician Assistant (PA), Certified Nurse Midwife (CNM)
• Current California BRN Furnishing Number (if NP or CNM)
• National Provider Identifier (NPI) number
• Must be within 50 miles of locations (relocation package offered!)
• Must possess reliable transportation or automobile transportation with a valid California Driver's License and insurance as determined by the requirements of each position
